This is an exciting time for Middlesbrough as we continue to develop ACT (Accessing Change Together) which integrates council staff and partner organisations and provides a range of support and interventions in relation to service users who need support with Housing Support, Homelessness, Domestic Abuse and Substance misuse. ACT is an integrated service, which means that lots of support services are working together and using a shared case load management system. This will make it easier for service users to get access to services they need so they receive the right support and easier for council staff and partner organisations to work together and share information to improve outcomes for service users. It is recognised that sometimes service users need help in several support areas. Working with a multi-disciplinary approach, the teams aim to react quickly to those needs, developing risk assessments and support plans to address those needs appropriately. The team will work alongside other services including Adult and Children’s Social Care and Public Health, to develop consistent pathways and effective processes. We have opportunities which have become available within ACT Front Door Team which is often the first point of contact for service users. We are looking to recruit staff who have the drive to help improve and contribute to the accessing change together vision, have experience of working effectively with a range of services and have the skills, passion and resilience to work creatively with service users who are accessing ACT and need ongoing support in relation to Housing, Homelessness and /or Domestic Abuse. Whilst in post there will be development opportunities alongside flexible working with the option of a blended working environment (home and office where appropriate).
