Job overview
An exciting opportunity has arisen to join Surrey and Sussex Health Care NHS Trust leading on Adult Epilepsy Care Pathways.
You will be part of the Neurology team, supporting adult epilepsy patients through assessment, treatment and support ensuring they receive high quality care.
You will support patients on the epilepsy pathway, from GP referral, through to treatment, follow up, support and discharge through personalised care planning.
You will be part of the MDT team, attending locally and central MDT meetings, and be required to present cases for review.
You will work alongside the paediatric epilepsy specialist nurse on transitional care from pediatric epilepsy into adult care pathways.
You will work with other appropriate professionals across all care settings of the pathway, building good relationships with both paediatric and adult epilepsy clinicians.
You will support patients in the outpatient and ward settings and will lead on managing the First Fit pathway for adults.
The post holder will be a registered nurse with a qualification and experience in the field of epilepsy and who is motivated and will adhere to the values of the service and Trust.
