Your role and responsibilities allocating and monitoring customer visits to frontline staff working in Home First responsible for implementing changes to the customer’s package of care on our TotalMobile platform, which sends work to our frontline staff working with Home First Team Leaders and Managers you will ensure the area runs smoothly and efficiently supported by the Allocation Team Leaders to regularly review capacity this is a fast-flowing service, and you will need to be responsive to accommodate workflow. You will need Level 2 in Maths and English ability to remain calm in stressful situations to be competent in use of email, the internet and other desktop applications communicate effectively, verbally over the phone to customers and frontline workers, and over email and Teams to the wider West Home First colleagues. The team Home First is a Suffolk County Council in-house provision for short-term reablement that offers care and an assessment service to individuals living in their own homes. The service accepts referrals directly from the community and through hospital discharges, with a primary goal of maximising clients' independence and reducing the likelihood of further hospital admissions. Supported by a diverse and multi-cultural team, Home First works collaboratively with colleagues across social care and health sectors.
