The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 110 physicians across 18 sites within London/Middlesex, Oxford, and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive collaborative primary health care to over 155,000 patients.
Family Health Team staff includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists, and other health care professionals who work with physicians as an interdisciplinary team, each using their experience and skills to provide patient-centred care. Requirements:
1. Academic preparation from an accredited Canadian university program in foods & nutrition or equivalent.
2. Practical training in an accredited practical program in Canada or equivalent.
3. Two to five years of clinical counseling and community nutrition experience or comparable skills, experience, and education.
4. Experience in program development, implementation, monitoring, and evaluation.
5. Member in good standing with the College of Dietitians.
Skills / Abilities:
* Ability to use a stepped care approach to service delivery, ensuring referrals are appropriately triaged to programming and/or community partners, reserving one-on-one appointments for high-risk patients or complex scenarios requiring individualized care.
* Ability to lead and support patient groups in a stepped-care model.
* Knowledge and proficiency in current, evidence-based methods and practices of nutrition care in primary care settings, with an emphasis on chronic disease management, health promotion, and risk reduction using concepts of goal-directed care and shared decision-making.
* Knowledge of social determinants of health, trauma-informed practice, and Indigenous cultural competency.
* Demonstrated organization, evaluation, time management, and interpersonal skills.
* Experience and commitment to working collaboratively with an interprofessional health team and community partners.
* Experience in supporting patients who are marginalized (physical, mental health, and social barriers) to access primary care.
* Demonstrated computer literacy to facilitate the use of electronic medical records, Ocean, Microsoft Word, Excel, Outlook, Teams, Zoom, and Webex.
* Desire to work in a continuous learning environment and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops, and conferences.
* Strong multi-tasking skills related to the delivery of efficient primary care including the ability to manage caseloads at multiple sites, coordinate service expectations between multiple sites, and balance direct/indirect patient care with administrative duties.
* Self-directed in marketing professional services and managing patient caseload.
* Excellent verbal and written communication skills.
* Ability to deliver evidence-based education and training to colleagues, staff, faculty, and learners, ensuring alignment with current best practices and research.
* Experience participating in quality improvement initiatives.
* Knowledge of the structure, goals, policies, and procedures of the organization.
* Demonstrated comfort with organizational change.
