Personalised Care Support Worker/ Administrator - Breast Team
We are recruiting for a Personalised Care Support Worker/ Administrator who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
You will be expected to be an experienced administrator who has excellent communication and IT skills to work with our Breast Cancer teams to implement Personalised care pathways for patients with Breast cancer.
Supporting Personalised Care Follow Up (PCFU), you will form an integral part of the multidisciplinary team, alongside the CNS caring. You will assist the MDT in coordinating all appointments, signposting patients as necessary to the relevant team member to support the patient through the self-managed part of their pathway.

Main duties of the job
1. Act as point of contact for queries relating to scheduling of patient appointments and coordination of patient follow-up pathways.
2. Respond to patient and other healthcare professionals' communication acting as a triage for patients and ensuring they are signposted to the appropriate member of the team.
3. Manage the database for all patients on the PCFU, liaising with the MDT coordinator to ensure patients are reviewed as necessary at the weekly MDT.
4. Be aware of the physical, emotional, and psychological needs of the patient. Able to observe, listen, and report any changes to the patient's condition to a member of the MDT so necessary guidance can be given.
5. Organise the survivorship workshops including organising the agenda, patient attendance, and relevant information being available to patients on the day.
6. Plan and organise your own workload, with guidance from the Breast Service Coordinator.
7. Enter patient data accurately into administrative systems ensuring this is undertaken in a timely manner.
8. Review cancelled/suspended/deferred patients ensuring appropriate action is taken after consultation with members of the MDT.
9. Ensure that all health records are tracked and securely stored according to departmental policy.
10. Provide general administrative support to the Breast Unit.
