This is a very exciting opportunity to step into a brand new role working for a visionary and mature Primary Care Network (PCN). The Learning Disabilities health check scheme is designed to encourage practices to identify all patients aged 14 and over with Learning Disabilities, to maintain a Learning Disabilities 'health check' register and offer them an annual health check which will include producing a health action plan. The Learning Disabilities health check scheme is one of several GP enhanced services. To support practices to achieve this, West Leeds Primary Care Network is looking for a suitably qualified Registered Nurse with an interest or expertise in Learning Disabilities.
Main duties of the job
The successful applicant will:
* Work alongside our PCN Learning Disabilities Care Co-ordinator and Cancer Care Co-ordinator, supporting practices as part of a multidisciplinary team to ensure patients receive the best quality care.
* Be responsible for the management of Learning Disabilities health reviews; this will include supporting the management team in the reviewing of clinical policy and procedure. The post holder will build relationships with wider external organisations to promote joined up patient care and services.
* Deliver clinic and home-based assessments and treatment plans including education and lifestyle advice.
About us
West Leeds Primary Care Network (WLPCN) provides enhanced clinical services across six GP surgeries within the Pudsey and Bramley area, with a total patient population of approximately 70k.
WLPCN currently benefits from the following clinical teams, directly employed by ourselves:
* Pharmacists and Pharmacy Technicians
* Health Care Assistants (Primary Care Support Workers)
* Learning Disability and Severe Mental Illness Care Co-ordinator
* Highly skilled prescribing Frailty and Home visiting team (Advanced Clinical Practitioners and Associate Clinical Practitioners)
We also have access to physiotherapy and mental wellbeing services.
Job responsibilities
The successful applicant will:
* Work with the PCN Learning Disabilities Care Co-ordinator to identify patients eligible for an annual health check.
* Ensure all documentation and information provided for this cohort is written and presented in an appropriate manner.
* Identify with patients if any reasonable adjustments are required to support them, and ensure these reasonable adjustments are documented in the clinical record.
* Support practices in delivering all the requirements of the annual health check which includes both physical and emotional checks.
* Following the review, formulate a care plan appropriate to the needs of the patient.
* Work in partnership with other groups in the development of innovative clinical practice.
* Identify whether the needs of the carer are being met and support or signpost as necessary.
* Work with the Cancer Care Co-ordinator to encourage patients to participate in cancer screening programmes.
* Facilitate and empower people with a Learning Disability to access other healthcare services.
* Support people with a Learning Disability within a range of settings to take control of their own healthcare. This may include creating or updating health passports.
* Where additional health and social care needs have been identified as part of the annual health check, to complete timely and appropriate referrals to the relevant specialist service.
* Follow the principles of the Mental Capacity Act 2005 when assessing capacity to gain valid informed consent. Where people lack the capacity to consent to treatment, work within a legal framework.
* Provide education and training to health care professionals across the PCN to enable better connection with this specific group of patients.
* Assist in managing a caseload, in line with PCN policies, guidelines and national standards.
* Promote and enable patient choice and independent living.

£45,000 to £51,000 a year depending on experience.
