Candidates must be willing to work on site in the hospital. THERE IS NO REMOTE WORKING FOR THIS POST.
£32 ph for 3 months. We are looking for an experienced Social Worker to work on site closely with Health colleagues to facilitate a safe return home for vulnerable adults who present in the Emergency Department at the Princess Royal University Hospital in Farnborough, Bromley.
Your responsibilities will include:
1. Completing assessments under the Care Act 2014 and Mental Capacity Act 2005.
2. Arranging care support to enable adults to return home and reduce the risk of social admissions to hospital or temporary placements.
3. Attending daily board meetings to discuss patient flow and plan the adult's return home.
4. Following up with the respective Locality teams after the adult has returned home.
You should have knowledge and experience in:
1. Facilitating safe hospital discharges.
2. Applying the Care Act 2014, including Section 42 Safeguarding threshold.
3. Conducting timely strength-based assessments.
4. Completing Mental Capacity assessments.
You will be supporting younger adults with physical disabilities and sensory impairments, as well as older people, to live as independently as possible and work with their carers to ensure risks are appropriately managed.
You need to be flexible, committed, and hardworking with a willingness to work to quality standards. You will be working within a multidisciplinary framework and must possess good communication and negotiating skills. Additionally, you will be required to develop close working relationships with partner agencies.
