Job summary
We currently have an exciting opportunity for a Remote based IP Clinical Pharmacist due to company growth and expansion to work full /part-time for one of our many PCN contracts across the UK. This opportunity isavailable for highly motivated and focused Clinical Pharmacists, who will beresponsible for the smooth running of day-to-day activities within generalpractice, whilst playing a vital role in providing clinical support and adviceto patients focusing on best practice and adhering to safety guidelines.
Full / Parttime hours can be worked within core opening hours of the GP practice.
Main duties of the job
1. As a clinical pharmacist you will confidentlyact within professional boundaries, supporting and working alongside a team ofpharmacists and GP's.
2. Resolve medication-related queries
3. Polypharmacymedication reviews and SMRs
4. Telephonegeneral medication reviews
5. Improve patient outcomes
6. Ensure better access to healthcare
7. Support practice staff with prescription andmedication queries
8. Medicine reconciliation and review post-discharge
9. Conductmedical related audits and work on service improvement with the practice
10. Provide expert advice on medications to otherclinicians and patients
11. Deliver clinical leadership on medicinesoptimisation and quality improvement
12. Ensure appropriate and efficient use ofmedication
13. Educate patients on medication and diseaseprevention
14. Anunderstanding of QOF, CQC and local incentive scheme workstream
15. Create and follow clinical pharmacy programmeaccording to practice policy
16. Excellent communication and interpersonal skills
17. Conductlong term condition reviews

Job responsibilities
The Role
As a remote Clinical Pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians.
You will help resolve medication-related issues, help utilise skill mix, improve patient outcomes and support the improvement of access to healthcare.
You will support practice staff with prescription and medication queries.
You will be required to work between the practice hours of and Monday to Friday, depending on contracted hours.
