We are looking to recruit a Family Intervention Worker for our Children’s Transitions Team, based at The Portal in Ellesmere Port.

The Children’s Transitions Team works with young people aged 0-25 years with disabilities. We work across the whole borough of Cheshire West and Chester, so there will be a requirement to travel.
The Family Intervention Worker role is wide and varied, involving direct work with young people and their families within their own homes and in the community, as we support them to transition from children’s social care to adult’s social care. This role will entail working from both a children’s and adult’s social care perspective. The Family Intervention Worker will support young people and their families to access appropriate services, promote independence, and understand the services available to them as they transition into adult’s social care. They will complete assessments of need, risk, and strengths within the family to enable social care teams to promote services/support relevant to the needs of our young people and their families.
