PCN Care Coordinator - Older Person's Integrated Care Team

To act as the single point of administrative and coordination support for the Older Persons Integrated Care (OPIC) Team within the Barnstaple Alliance Primary Care Network. This role, alongside the OPIC Team clinicians, will work closely with the member practices to provide care to our care home and housebound patients registered within the PCN.
Hours available are 22.5hrs a week, worked over 4 or 5 days.
Main duties of the job
The OPIC Team consists of Nurse Practitioners and a GP Clinical Lead. The PCN Care Coordinator role is pivotal to ensuring all patients receive the best possible care and service through coordinating and supporting the delivery of all key activities of the OPIC Team clinicians. The role will involve coordination of home visits for housebound and care home patients. The role will also include supporting and coordinating focused work around the delivery of the Quality and Outcomes Framework (QOF) and specifications within the Network Contract Directed Enhanced Service (DES), including Enhanced Health in Care Homes (EHCH) and Structured Medication Reviews (SMRs).
Job responsibilities
MAIN DUTIES & RESPONSIBILITIES
* Provide efficient and comprehensive administrative and clerical support to the OPIC Team on a day-to-day basis.
* Coordinate and arrange Long Term Condition reviews and vaccinations for both Care Home and Housebound patients for the OPIC team.
* Continuously monitor, manage, and prioritise all incoming and outgoing correspondence and workflow for the OPIC Team (such as the OPIC team's email inbox and TPP SystmOne task boxes), initiating responses where appropriate.
* Being the single point of contact for the OPIC team for the member practices and teams within the Barnstaple Alliance PCN, Care Homes and other healthcare professionals.
* To work with the OPIC Team to identify and support the organisation of patient caseloads with Care Homes and the Cared for at Home.
* Support the OPIC Team with inputting data and maintaining patient records and patient lists.
* Practical application of TPP SystmOne and Microsoft Teams for coordinating workload, organising individual team members caseloads and appointments, and developing reporting mechanisms.
* Coordinate agendas, prepare patient lists and take minutes of key meetings including relevant Multidisciplinary Team meetings.
* Produce via TPP SystmOne and other digital tools reports to support the work of the team.
* Support the PCN, its member practices and the Clinical Lead of the OPIC Team with aspects of the Network Contract Directed Enhanced Service (DES). Specifically, the Impact and Investment Fund (IIF), including the Enhanced Health in Care Homes (EHCH) and Structured Medication Review (SMR) areas.
* Coordinate Care Home Ward Rounds, liaising with staff at all relevant organisations and booking patients into the Weekly Care Home Round appointment template within TPP SystmOne as needed.
* To help Care Home Managers and GP Practices address their patients needs through answering queries, managing diaries and appointments, and delivering care plans.
* To support the OPIC Team in the development of robust care plans in line with best practice.
* Organising which elements of clinical (and QoF) checks need applying on visits.
* Support and coordinate work around vaccinations for the PCN as needed (including reporting, invitations and data input).
* Coordinate both acute and chronic visits for the OPIC Team.
* Order medical and pathology supplies as needed for the OPIC team.
* Work with the PCN Operational Manager to plan and coordinate the OPIC Team's activity including holidays and sickness.
Person Specification
Qualifications
* Educated to GCSE level or equivalent
Experience
* Have experience of working with the general public
* Have experience of administrative duties
* Have experience of working in teams and be tolerant of other skills and abilities
* Have experience of working in a primary healthcare setting ideally with some experience of working with a PCN team
* Experience of working in or with a Multidisciplinary Team
* Experience of coordinating a clinical team or tasks

* Excellent interpersonal skills
