We have an exciting opportunity for 2 candidates to join us in our expanding 7 day Frailty journey.
This post will be working across Worcester Royal Hospital and Alexandra Hospital in Redditch.
Shift patterns consist of long days 7:30am to 8:00pm with a 60 minute break. This post include weekend working.
We are open to full time and part time positions.

Main duties of the job
Toensure cohesive working between Acute and Community Services
Tocontribute to the organisation of frailty multidisciplinary, patient centredcare
Tofacilitate flow of patients both from admission via A&E/MAU and through thewhole patient journey.
Toprovide expert relevant clinical advice and support to other healthprofessionals, patients and their carers
Tosupport the development and implementation of best practice within thespeciality of frailty medicine
To provide a specialist nursing role in theassessment and management of patients who are frail.
To provide an outreach service to other patients inthe Hospital who have a high frailty score
To provide key support and an educational resourcefor any qualified and unqualified nursing staff, allied health professionals, junior doctors and students working within the speciality
Todevelop nurse led frailty follow up telephone advice after discharge.
Toplay a key role in ensuring that national and local targets for frailtypatients are met.
