Job Description
About the role
An exciting new opportunity has become available for a Care Partner to join our Homecare team, with travel throughout the Moreton Bay North suburbs, including Caboolture and Bribie Island. This position includes a car allowance to support the required travel.
Reporting to the Senior Care Partner, the Care Partner will champion and support the needs of new and existing Aged Care Home Care package clients. The successful applicant will have the ability to optimise client experience through diverse and individualised service options, while offering collaborative and integrated services to proactively meet client priorities. The Care Partner will ensure new and existing plans meet the needs of each individual, adhere to legislative requirements, and ensure our clients feel welcomed and part of the BallyCara family at all times.
The ideal candidate will have extensive experience in customer service and/or case management and will be professional and friendly in their approach. You will be self-motivated and able to work autonomously as well as in a team environment. This is a part-time four day per week position, with work-from-home opportunities available for the right candidate.

Desired Skills and Experience
To be successful in this role, you will have:
* Education, training and/or experience equivalent to a degree in Administration, Sales, Business, Nursing and/or Allied Health
* Effective communication and time management skills
* Optimal case management skills
* Excellent communication skills, verbally and in writing, including the preparation of reports
* Proven experience and ability to build positive and productive relationships with well-developed interpersonal skills, negotiating, influencing and facilitation skills.
* Broad knowledge of the Aged Care Industry.
* Experience in excellent customer service.
* Strong computer skills with intermediate-level knowledge and application of MS Outlook, Word, and Excel
* Some knowledge of Contract Administration and ability to read through Agreements.
