Main duties of the job
In accordance with the practice timetable, asagreed, the post-holder will make themselves available to undertake a varietyof duties, including surgery consultations, telephone consultations andqueries, visiting patients at home, checking and signing repeat prescriptionsand dealing with queries, paperwork and correspondence in a timely fashion.Making professional, autonomous decisions inrelation to presenting problems, whether self-referred or referred from otherhealth care workers within the organisation. The GP Partners hold key leadership positions in the PCN andthe ICS and can provide a wealth of opportunity and experience to other membersof the team. The practice is heavily involved in supporting and developing PCNservices across the whole of Erewash.
We are part of Erewash Health Partnership and have access toa wide range of networked services including home visiting, care home supportteam, pharmacists, social prescribers and first contact physios. We offer allcore services as well as minor surgery and contraceptive services.
Education remains an important part of the ethos of the practice,and we train registrars, medical students, non-medical prescribers as well assupporting apprentices from the local community. Job responsibilities
Assessing the health care needs of patients withundifferentiated and undiagnosed problems
Screening patients for disease risk factors andearly signs of illness
Developing care plans for health in consultationwith patients and in line with current practice disease management protocols
Providing counselling and health education
Admitting or discharging patients to and from thecaseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultationnotes to agreed standards
Compiling and issuing computer-generated acute andrepeat prescriptions (avoiding hand-written prescriptions whenever possible)
Prescribing in accordance with the practiceprescribing formulary (or generically) whenever this is clinically appropriate
In general, the post-holder will be expected toundertake all the normal duties and responsibilities associated with a GPworking within primary care
Awareness of and compliance with all relevant practicepolicies / guidelines, e.g. prescribing, confidentiality, data protection,health and safety
A commitment to life-long learning and audit to ensureevidence-based best practice
Contributing to evaluation / audit and clinical standardsetting within the organisation
Contributing to the development of computer-based patientrecords
Contributing to the summarising of patient records andread-coding patient data
Attending training and events organised by the practice, PCNor other agencies, where appropriate

Experience
* Telephone triage

* Experience of working as duty doctor
* Fully qualified GP
* Medical education
* Proven track record of delivering QOF
* Higher postgraduate qualifications
Qualifications
* Knowledge of GP contracts/enhanced services/QOF
* Knowledge and completion of CPD to remain up to date with GP appraisal/revalidation
