Job Purpose To assess, plan and develop treatment programmes for clients with a wide variety of complex physical needs in a clinic setting. Determine clinical diagnosis and treatment indicated and maintain records as an autonomous practitioner, working without direct supervision. Be responsible for continuing the on-going development of high standards of clinical practice and expertise within the service, contributing to implementation of specific changes to practice and service protocols. To effectively triage MSK self-referrals, using risk stratification tools and place the patient in the appropriate treatment pathway i.e self-management using relevant MSK rehabilitation tools, virtual or F2F assessment. Dimensions To be responsible for the delivery of the Musculoskeletal physiotherapy within the MOSAIC service (Musculoskeletal Pain, Orthopaedic, Spinal and Integrated CFS/ME). Clients are referred by their GP, other health professionals, social services or self-referral. Dependent on your experience, your role may include the delivery of pain management programmes and the assessment of complex persistent pain patients. To supervise less experienced members of staff and students. To supervise, educate and contribute to the assessment of undergraduate physiotherapy students if required, ensuring the standard of practice and teaching meets the standards set by the degree level qualification. Administering and evaluating outcomes of injection therapy with guidance from Advanced Pain Practitioners. To work in a variety of clinical environments, including potential lone working, and liaise with the extended health team as required e.g. GPs, other health professionals within and outside of CityCare. To monitor the effect of treatment delivered by yourself and other members of the team, including the outcome of injection therapy. To monitor and maintain performance of clinical sub-team of physiotherapists. To work alongside the MOSAIC Administrative Team to maintain an efficient service for patients, delivering excellent customer service and contributing to the overall performance of the service. Key Responsibilities Clinical To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care and to support more junior staff to do likewise. To have a lead role and determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each client. To undertake the comprehensive, advanced assessment of clients including those with complex and/or chronic presentation using investigative and analytical skills and to formulate the individualised management and treatment plans, using clinical reasoning, a wide range of treatment skills and options to devise a specialised, individualised programme of care. To accept clinical responsibility for a designated caseload of patients and to organise this effectively and efficiently, with regard to clinical priorities and time management. To provide highly specialised advice, teaching and training to other members of the primary health care team regarding pain management of clients with problems related to rehabilitation, ageing and obesity. To educate, supervise and manage less experienced members of the team along with support staff and students. Using appropriate documentation. To assess capacity, gain informed consent and work within a legal framework with clients who lack the capacity to make informed decisions. To provide spontaneous and planned advice, teaching and instruction to patients, carers and other professionals to promote an understanding of the aims of pain management and to ensure a consistent approach to client care. To work as a lone practitioner with supervision from a Band 7 Physiotherapist or Pain Practitioner. The Team Manager will also provide performance supervision. Professional To participate in the development of good practice for Physiotherapists within Nottingham CityCare Partnership under the guidance of the AHP Professional Lead. To demonstrate a commitment to evidence-based practice. To be responsible for own Continuing Professional Development, under the supervision of the band 7 physiotherapist, keeping abreast of new trends and developments and incorporating them into your work as necessary. Attend relevant conferences and internal/external training courses. Support more junior staff to do likewise. To maintain a high level of physiotherapy standards and work to the Physiotherapy Code of Professional Conduct. To actively participate in the services training strategy, including in-service training and multi-agency training. To participate by attendance and presentation at staff meetings, tutorials and training sessions. To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to the work situation. To assist with development and participation in the services audit projects, evidence based practice projects and outcome measures, either individually or with other senior therapists. To make recommendations for change. To participate in the staff appraisal scheme and Personal Development plans as both appraiser and appraisee. Organisational To support the Team Manager in implementing targets and objectives to assist in the on-going development of the Team. To participate in service evaluation and advise the Team Manager on the service needs of the client population. To be responsible for management duties when requested by senior clinical staff. To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership CIC. To manage the day-to-day delivery of the physiotherapy service within own clinics. Also organising own diary and time management. (Arranging assessments and appointments for own caseload). To ensure the health & safety of staff and clients at all times in accordance with Health & Safety legislation and Organisational policy and procedures. To adhere to all other Organisational Policies and Procedures. To attend team meetings. To use information technology on a daily basis and participate in identified data collection and statistics and to maintain clear, concise and timely records in line with the service policy.
