Job summary
We are seeking a highly motivated enthusiastic and passionate learning disability nurse to join the Adult Learning Disability (ALD) Durham & Darlington Health Facilitation Team. The team is in the early stages of merging with the Tees Enhanced Physical Health Team and will become Durham Tees Valley (DTV) ALD Physical Health Team.
This is an exciting time to be part of the clinical transformation with the aim of providing an equitable service across the locality.
The Team provides specialist advice and support to both people with a learning disability and to health services to reduce the health inequalities and premature mortality that people with a learning disability role is a full-time position working hours Monday-Friday ied nurses are expected to participate in the Adult Learning Disability Nurse on call rota (Monday-Sunday 5pm-9am)
Base is negotiable with an option of Greenlane, Spennymoor or Hundens Lane, Darlington.
Main duties of the job
The successful candidate will work with health colleagues within the wider Learning Disability Community Teams, Local Authority colleagues and in liaison / partnership with primary and secondary care services, using a Person-Centred Approach to enable and support adults with a learning disability and their carers to promote the health needs of people with a learning disability and enable access to wider health services.
The main responsibility of the role: o Supporting with a wide range of health promotion projects o Supporting with reasonable adjustments to access right care and support in relation to physical health. o Training and education for services and carers. o One to one support for patients who have changes in physical health and/or need support to navigate health services. o Using contemporaneous evidence base and knowledge of health systems to navigate patient journey with regards to new or existing diagnosis. o Lead professional for a complex caseload and supporting and supervising junior members of the team The successful candidate with be expected to support in the delivery of the additional functions of the team. This includes the implementation of National NHS England work programmes

Qualifications
Essential
1. Current professional registration with Nursing & Midwifery Council - RNMH or RNLD (RNSC - CAMHS only ) - RGN (If appropriate)
2. Evidence of Continuing Professional Development
3. Recognised Practice Supervisor and/or Practice Assessor, clinical educator or equivalent. Must be achieved within 6 months
4. Trained in Clinical Supervision and providing clinical supervision OR willingness to work towards within 6 months
Experience
Essential
5. 18 months post qualifying experience including sign off of appropriate preceptorship
6. Supervising or assessing students
7. Providing clinical supervision to individuals or groups as professionally appropriate
8. Experience of working with people with Learning Disabilities
9. Working collaboratively with service users and their families/carers
10. Working in a multi-disciplinary team
