Carlton Court hospital is a satellite site of The Jame Paget University Hospital, it is situated further down the East Coast in Carlton Colville, Suffolk. Main duties of the job Carlton Court is made up of 3 separate units which all have 11 single rooms in each, along with a day room. There are situated on beautiful grounds and communal gardens which are open to staff and patients. Carlton Court supports patients who are medically optimised to leave hospital but requiring on going management from either the therapy team with a reablement pathway or from social prescribing within the integrated discharge team. Registered Nurses at Carlton Court are expected to liaise with all members of the MDT, acute and community based to ensure a smooth transition from hospital back into the community. Regular meetings are held with the integrated discharge team to ensure that patient are going to safe and able to remain out of hospital with the right support and care they require. Applicants must have excellent communication skills and be committed to providing a high standard of patient care whilst working well as part of the multi-disciplinary team. Detailed Job Description And Main Responsibilities The post holder is responsible for the assessment of care needs and the development implementation and evaluation of programmes of care. Ensure that agreed standards of nursing practice within the ward/department area are maintained. To keep abreast of current trends and developments within nursing, being constantly aware of the need for self-development. Person specification Education And Qualifications Essential criteria Registered nurse with the Nursing and Midwifery Council (NMC Desirable criteria Undertaken Supervisor and Assessor training. Experience And Knowledge Essential criteria Relevant experience in past 12 – 18 months of looking after patients in a healthcare environment.
