Main Base: Textile Hall, Market Street Bury (across site working will be required on occasion)

Together - we will support you to develop your career providing a wealth of training, resources, and opportunities. The Brokerage Team is an integral part of the assessment and care management process and whole system flow and performance.
About our team and the expectations of your role
The main function of the Brokerage team is to support access to the Right Care, at the Right Time, in the Right Place for all customers who are referred to our service. You will be expected to support scrutiny of assessments referred to us to ensure that Strengths based principles are part of our core, prior to working with our commissioned care providers to put the care in place within required timescales. You will support Home First Principles and provide information on capacity and demand as well as offer advice and guidance on market position to our departments.
You will support system flow and performance by ensuring that there is movement in our home and bed-based services within the Intermediate Tier, Rapid Response, Connect and Direct, and customers who are discharging from acute hospital services.
You will also be supporting people to ensure needs are met and support our Integrated Neighbourhood Teams to put care in place to meet needs that are assessed.
You will support payments to providers by maintaining input to our finance system and validate appropriate funding routes.
You will be adaptable, with a let’s do attitude towards your work and our team role which we deliver together our customers' local needs. And your actions will support your commitment to the team and the organisation.
Team Dynamics
Brokerage is a small, but perfectly formed friendly and supportive team that has a lot of impact and is responsive to meet the needs of customers and assessment and care management teams. You will have support from 2 other Brokerage Assistants to develop in your role, a brokerage supervisor and the Integrated Discharge Services Manager that heads up the team.
About you
As brokerage is a bespoke service within Bury with its own processes, we know that a significant induction is important to you and we have learned that the skills and strength needed for the role are from people who can evidence their experience and knowledge in the following areas:
1. Operational Health and Social Care Assessment and Delivery experience with the ability to scrutinise assessments in line with a Strengths Based approach within contractual terms and conditions, with a Health or Social Care Qualification or related and transferable experience for the role.
2. Able to build and develop trusting professional and responsive relationships with a varied customer base from people and families who require services, Care Providers, social work and assessment and care management teams across the footprint of Bury from our front door (Connect and Direct) Admission avoidance teams such as (Rapid Response) Assessment and Care Management within our Integrated Neighbourhood Teams in each locality, & Review Team, Rehabilitation teams (Intermediate Care Tier) IMC, and support Mental Health and Learning Disability colleagues, Integrated Discharge team at the hospital by ensuring people receive the Right care at the right time in the right place with a Home First Approach both for Care at Home (Homecare) and Care Homes currently. We do anticipate that this may expand into other services in the future.
3. Experience of working within the Care Act, Mental Health Capacity, Deprivation of Liberties in care homes and in domestic settings, Safeguarding Adults.
4. Ability to use software and Information technology systems and processes and learn new skills at pace.
5. Good Communication skills with sound experience to support and advise teams and individuals and support professional discussions.
6. Experience of using finance systems to ensure providers are paid in a timely manner and scrutinise costs and to complete basic calculations.
7. Written and verbal skills that comply with the councils GDPR, Confidentiality, Data Protection, Governance regulations and supportive of CQC regulations.
8. Be a reliable and supportive member of the team and uphold our values and be willing to develop.
9. Able to evidence a clear DBS for Adults.
