Role Overview
You will be responsible for supporting the Service and Deputy Manager and be the point of contact for the rest of the team when the Locality Manager is off-site. You will carry out care needs assessments, create and maintain person-centred support plans, and ensure high-quality support is provided to each of the people that we support along with the staff teams. You will promote good practice and act as an effective and positive role model. Your working hours will be a mix of on-rota and off-rota time, and you may be required to provide support in any location appropriate to meet the needs of the service.
Key Responsibilities
* Overall responsibility for the development and implementation of individual support plans, including assessments and relevant risk assessments for new people we support.
* Encourage the people you support to take an active role in daily living and ensure the appropriate level of individual care and support is identified.
* Manage the review and maintenance of all support plans, ensuring the ongoing suitability of the support provided, including daily outings, holidays, or recreational pursuits.
* Assist individuals with personal care and hygiene, including moving techniques, using hoists, and repositioning individuals as required.
* Plan and arrange individuals' daily outings, holidays, or recreational pursuits.
* Build working relationships and engage in positive communication with individuals, families, advocates, and other professional colleagues.
* Assist individuals with household activities such as cooking, mealtimes, laundering, and cleaning.
* Record and complete appropriate documentation to ensure accuracy and compliance, including compiling reports for contractual or company compliance.
* Ensure that all people we support have up-to-date medications and MAR charts per the company’s Administration of Medication Policy.
* Support staff through coordination and completion of supervision, appraisal, and team meetings, including recruitment and induction of new staff.
* Participate in relevant multi-disciplinary meetings for the people that we support.
* Ensure high standards of care and support are delivered through proactive auditing.
* Organise and contribute to team meetings to promote communication within the team.
* Ensure good governance through participation and completion of the auditing process.
* Support and manage staff teams through good leadership and management.
* Undertake all relevant training to professionally develop and gain skills beneficial to the individuals.
* Follow and understand all policies and procedures applicable to the service and the company.
* Work shifts and visits on a staffing rota, including weekdays, weekends, split shifts, wake nights, sleep-ins, and bank holidays as required.
You may be required to undertake other duties commensurate with your role and/or working hours, as may reasonably be required at other sites or locations in the community.

Essential Qualifications
* Willingness to work towards NVQ in Care & Management
* NVQ3/SVQ3 in Care (or equivalent)
