This is a Joint vacancy for both Aintree Core24 and Southport Core 24.
Southport and Aintree Core 24 are specialist assessment services for service users' mental health who present to the Emergency Department in times of crisis or are referred from the acute general hospital wards. This includes the formulation of treatment and risk management plans based on assessment of needs with appropriate triage and diversion from the service.
The service operates on a shift system of long days, shorter nine and half hour shifts, and nights to provide a 24/7 service throughout the year. The service has recently undergone organisational change to enhance flexibility across the Urgent Care pathway, covering Urgent Care 24/7, as well as supporting Royal, Warrington Whiston, and Southport Core 24 services when required.
Responsibilities:
1. Provide high quality, evidence-based care within the division for service users and their carers in inpatient and community services.
2. Work in a socially inclusive way and in line with the recovery model.
3. Function as named nurse/care coordinator for service users, undertaking assessment, planning, evaluation, and review of care within a recovery-focused model.
4. Participate as an active member of the multi-disciplinary team.
5. Provide advice, expertise, and leadership within the team.
6. Supervise care for service users presenting with complex and high-risk needs.
7. Work autonomously, making clinically appropriate decisions based on information received and knowledge from the multi-disciplinary team.
8. Act as a mentor for students.
9. Coordinate the care of service users, carrying out assessment planning, implementation, and evaluation of care under the care programme approach, supervised by the line manager.
10. Ensure that the service users' care plan is developed to meet their assessed needs and that unqualified staff deliver care as identified in the care plan.
11. Liaise with family members, carers, and significant others when completing assessments and developing care plans.
12. Undertake and supervise risk assessments and formulate risk management plans for service users, considering contributions from other team members and family.
13. Ensure safeguarding requirements are followed and participate in meetings as appropriate.
14. Direct unqualified staff to ensure delivery of high standards of care to service users.
15. Participate in setting standards for care based on sound evidence and research findings.
16. Participate in the systematic monitoring and evaluation of service users.
17. Communicate relevant information to other members of the multi-disciplinary team that impacts service users’ care.
18. Use verbal and non-verbal communication skills to impart sensitive information.
