Job summary We are seeking to recruit experienced part time community staff nurses to work in the District Nursing Teams based at Clarendon, Hebburn. The post holder will work within the District Nursing Teams, providing care to patients in their own homes / care home / other care settings by providing assessment and treatment of patients in the community over an 8am 6pm period, 7 days a week. You will also support the appropriate early discharge of patients and will give support to Primary Care and the Unplanned Care Team. We are recruiting part time posts (22.5 and 30 hours per week).
