Job summary
North Watford Primary Care Network has an exciting opportunity for a PCN advanced nurse practitioner to join their well-established team, on a full-time, permanent basis.
Main duties of the job
1. Develop, implement and embed health promotion and wellbeing programmes.
2. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
3. Implement and evaluate individual specialised treatment plans for chronic disease patients.
4. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health.
5. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
6. Provide wound care (ulcer/Doppler etc).
7. Provide specialist clinics such as ear syringing, contraception etc.
8. Undertake the collection of pathological specimens.
9. Provide travel medicine services.
10. Request pathology services as necessary.
11. Process and interpret pathology and other test results as required.
12. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.

Qualifications
Essential
13. Registered Nurse with Nursing and Midwifery Council.
14. Qualified Independent Nurse Prescriber on the NMC register.
15. Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
16. Minor illness qualification.
Experience
Essential
17. Venepuncture, ECGs, new patient medicals, chaperon procedure, diabetes, hypertension, CHD, women's health.
18. Experience of working as a practice nurse or community nurse.
19. Wound care/removal of sutures and staples.
