Responsibilities:
* Conduct comprehensive reviews and reassessments of care packages for adults.
* Develop and implement care plans tailored to individual needs.
* Work collaboratively with service users, families, and other professionals to provide holistic support.
* Maintain accurate and up-to-date records in line with statutory and local requirements.
* Ensure all actions are compliant with relevant legislation, policies, and procedures.
* Advocate for the rights and needs of service users.
* Participate in team meetings and contribute to service development initiatives.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
