Interim Social Worker - Hospital Social Work Team

Pay Rate: £20.85 per hour PAYE
Hours per week: 37 Monday – Friday, normal working hours
Role Length: This opening assignment is for 4 months

This position involves hybrid working but requires someone who can respond to urgent requests to visit anywhere in the borough. The role includes a combination of home working, office-based, and hospital-based activities.
The ideal candidate will have a good understanding of the current Discharge to Assess Process in relation to assessing/supporting people when they leave the hospital. It is essential to have experience working collaboratively with health colleagues, as the role entails working in partnership with health in various acute and intermediate care settings. Responsibilities include the completion of Care Act and Mental Capacity Assessments, alongside best interest decision-making. The role will involve undertaking safeguarding investigations, so knowledge of the safeguarding process is essential. Significant input in complex discharge planning with health colleagues is required, ensuring the right pathways are achieved for individuals while promoting a strengths-based approach as standard.
Qualifications:
* Social Work qualified
* HCPC Registered
Knowledge:
* Demonstrate a sound working knowledge of Adult Social Care legislation and practice.
* Sound knowledge and confidence of the Care Act 2014, the Mental Capacity Act 2005, and the Human Rights Act 1998.
Experience:
* Relevant experience of working with adults in community and/or hospital settings.
* At least 12 months experience post newly qualified social worker status.
* Working across a range of agencies and understanding their roles.

* Excellent communication skills with a wide range of people and mediums.
* Good team and collaborative working skills.
* Effective organisational skills and ability to set priorities.

* Ability to use the electronic client recording system and mobile working tools.
* Ability to undertake safeguarding enquiries.
* Ability to use the Mental Capacity Act (2005) to determine an adult’s capacity and to consider making best interest decisions where appropriate.
* Ability to listen and work with adults in a person-centred approach.
