Job overview
1. To plan and manage own workload, which will be identified and supervised by Senior Community Nurse for Children with a Learning Disability and CNCLD Team Lead.
2. To share responsibility for day-to-day management of the service in the absence of the Team Lead and other team members.
3. To be a competent, autonomous community practitioner, with support from senior members of the team.
4. To work with a team of nurses and as a member of the Children and Young Peoples service to promote a seamless provision for children.
Main duties of the job
1. To provide care in partnership with parents/carers/child using interventions appropriate to the assessed need as per the CNCLD Standard Operating Procedures.
2. To evaluate the outcome of interventions using a process of regular review in collaboration with the child, family, CNCLD and other professionals involved.
3. To advise on the promotion of health and the prevention of ill health.
4. Participate in Safeguarding Children processes as required, sharing information as necessary and completing reports within timescales.

Person specification
EDUCATION, QUALIFICATIONS & TRAINING
* NMC Registered Nurse – Learning Disability, Mental Health, Children or Adult.
* Evidence of some relevant post registration education.
EXPERIENCE
* Experience of working with families with children with learning disabilities.
* Evidence of providing complex clinical assessment to determine health care need for children/people with learning disabilities.
* Evidence of planning effective care with children/adults with complex needs.
* Evidence of effectively implementing planned care with families of children/people with a learning disability.
* Evidence of being able to effectively demonstrate the voice of the child when evaluating the provision of care.
* Can demonstrate working to scope of professional practice and use of evidence-based practice.
* Experience of Safeguarding Children Policies and Procedures.
* Experience in multi-agency joint working practice and some experience of working with children who have challenging behaviour.
* Demonstrate an awareness of Mental Capacity Assessments/Best interests assessments.
SKILLS, ABILITIES & KNOWLEDGE
* Clear, concise verbal and written communication skills.
* Experience of working as part of a multi-skilled team and appropriate delegation of tasks and supervision.
* Excellent computer skills on a range of software applications.
* Ability to write detailed reports/referrals so that the child or young person can access the appropriate care and support.

SPECIAL CIRCUMSTANCES
* Ability to travel in a car to other sites to various community sites on a daily basis.
* Ability to manage small loads and transport equipment.
