Accept and take responsibility for referrals in respect of the Early Intervention team.
Take responsibility for own caseload.
Assess for suitability for reablement programmes and write the programmes as appropriate.
Assess and recommend equipment and adaptations and Assistive Technology solutions.
Maintain accurate, comprehensive and contemporaneous documentation, in line with legal and service requirements.
Undertake peer clinical supervision groups of unqualified staff under the guidance of the manager.
Provide assessment, treatment and management of babies, children, adolescents and adults who present diverse and complex physical, developmental and learning needs.
Be aware of legislation and policies affecting Occupational Therapy in adults and children’s services practice.
Provide a specialist response in complex cases, formulating and implementing intervention plans aimed at reducing the impact of disability, increasing independence for children and adults in their own homes through the provision of equipment and adaptations.
Ensure that there is a planned integrated approach to the provision of equipment and adaptations, taking into account the identified Occupational Therapy goals and priorities.
Work closely with social workers, family, key workers and technical staff and other relevant professionals to inform analysis and reasoning where appropriate.
Provide assessments for adaptations in the home environment for children/adults where the integrated approach and care plan indicates no further rehabilitation potential and/or a compensatory approach is needed for the child/adult to reach their full potential.
Attend case conferences, reviews and meetings as required by clinical or managerial responsibility.
Provide specialist advice to members of the service regarding the management of patients with complex needs to ensure holistic care.
Discharge cases appropriately, agreeing a point of closure with the parents and child and informing other professionals as required.
Ensure you operate within GDPR guidelines by regularly reviewing data held and destroying information in line with retention schedules.
