Exciting opportunity to join a thriving, developing service as a B6 team leader. This will mean overseeing the shift and managing the staff, referrals and prioritising visits all over the Torbay and South Devon area for our patients in their home residence, preventing hospital admissions and aiding hospital discharges. We have the use of 3 pool cars.
We are a friendly, highly adaptable team to deal with numerous issues we face whilst working at night.
Regular supervision sessions, both individual and group, that we can all contribute to, so everyone is heard, and we can all learn.
Your new role will be providing holistic nursing assessment, high quality care, support, advice, and information to patients and carers with the aim of maintaining patients in their own homes, enabling them to enjoy maximum independence and optimum quality of life.
Ensuring the effective co-ordination and provision of the nursing service in the absence of the Zone Nurse Lead and actively participate in the assessment, prioritisation, planning and discharge of complex patients from secondary to primary care.
Main duties of the job
* To communicate formally and informally with the zone DN Team Leader, to ensure best practice and consistency with the community and intermediate care service in Torbay Care Trust.
* To communicate fully and in depth with the multidisciplinary team discussing patient treatment options and progress.
* To communicate and liaise effectively with the integrated health and social care team, voluntary agencies, secondary care, G.P.'s, carers and relatives.
* To use set transfer and discharge protocols.
* To complete regular supervision sessions, both individual and group.
* Working across the Intermediate care and community nursing teams to provide crisis response nursing interventions, assessment, liaison, referral, co-ordination, delegation of care, or placement to prevent hospital admissions. Aiding hospital discharges to ensure patient safety and providing specialist clinical advice to the multi-disciplinary team.

Qualifications
* Qualified Registered Nurse Level 1.
* Bsc (hons) in Community Health Nursing (District) or equivalent or working towards.
* Proven experience in leadership skills.
* Able to work as an autonomous practitioner.
* Proven relevant I.T. knowledge and experience.
* Evidence of ongoing training and development.
* Holistic client assessment skills using an identified model of care.
* Ability to make sound clinical judgements and decisions in response to complex or crisis situations.
* Able to work as an autonomous practitioner.
* Awareness of budget management skills.
* V300 Independent/Supplementary Prescriber.
* Comprehensive understanding of Health Care services.
* Health and Safety issues, Risk assessments.
