Job summary The post holder is an autonomous specialist practitioner responsible for providing professional management of patients as part of the Sleep/NIV service. The post holder is expected to promote excellent standards of care for patients by contributing to the future developments of the service. Establish an effective workplace culture within the sleep/NIV service across the health economy that sustains person-centred, safe and effective care through self awareness, active learning, development, improvement and innovation The post-holder will support the Ward Sisters/Charge Nurses, Senior Matron and Matron for the respiratory wards and outpatient departments promoting excellence in care to maintain and improve clinical standards. The post-holder will ensure that all patients are treated with compassion, dignity and respect, placing value on the diversity of the local community and beyond. They will promote best practice in the prevention of infection through ensuring a clean and safe environment, and that clinical practices are in accordance with Trust Policy and best practice guidelines to minimise the risk of infection. The post-holder will work with the multi-professional respiratory team in the implementation and delivery of an innovative and evidence-based service that addresses the needs of patients with acute or chronic respiratory/sleep disorders and their carer's. Main duties of the job To organise and manage own caseload Line manages Sleep/NIV associate practitioners Developing a learning culture and facilitate interdisciplinary learning across respiratory medicine Take responsibility for the day-to-day management of respiratory /sleep/NIV patients within resource limitations. Is an advanced autonomous practitioner who undertakes a comprehensive assessment of all patients referred to respiratory/sleep medicine from both outpatinet and inpatient settings. Includes assessments of medical, functional, social and mental status. Develops a care plan and undertakes and co-ordinates its implementation, monitoring and evaluation. Is an autonomous specialist practitioner working within defined protocols, delivering evidence based care to a variety of patients with respiratory/sleep conditions. Is highly competent to an advanced level in the use/application of specialised respiratory equipment, including non-invasive ventilation, continuous positive airway pressure. Is proactively involved with the coordination and management of patients requiring investigation and subsequent treatment. Uses results from medical assessment tools such as multidisciplinary notes, x-rays, arterial and capillary blood gases, sputum cultures, blood tests and sleep studies to help inform clinical decision-making.
