Job summary Job Purpose: As a self-directed individual this role will involve coaching and motivating patients in managing their own health and wellbeing and helping them to live independently and improve their own health outcomes. The post holder works with individuals and/or groups of patients to deliver sessions on healthy eating, physical activity and health and well-being to meet personalised care plans and health goals. You will use a holistic approach to promote lifestyle approaches and support patients to address existing health problems and prevent new ones. Health coaching uses coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access/ utilise community support offers. You will work with your clinical and non-clinical colleagues across the PCN to ensure the role delivers the best possible outcomes for our patients. The role includes supporting self-management education, peer support, case management and facilitating group consultations. Your approach will be non-judgemental, based on strong communication and negotiation skills. You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions. Your role and skills support and encourage the prevention of developing further illness, or the deterioration of existing long-term conditions. Main duties of the job Health and Wellbeing Coaches support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them. They support people to develop their knowledge, skills and confidence in managing their health and care, to improve their health outcomes and quality of life and support them making changes in health-related behaviours. Health coaches do this by coaching and motivating people through multiple sessions to identify their needs and to set goals for themselves, and through providing access to interventions such as self-management education and peer support. Health and Wellbeing Coaches manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload. They identify when a persons needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations.
