Shift Patterns: Monday to Friday, 9:00 AM - 5:00 PM

What you can add:
* Provide a holistic patient assessment and structured carer interview.
* Complete specialist initial assessment and interpretation of results to provide a baseline for future treatment/care.
* Work autonomously and show initiative when planning clients' care.
* Deliver diagnosis and offer expert advice regarding the diagnosis and treatment and provide education/support to both clients and carers.
* Provide education on the different kinds of dementia and how to manage the symptoms of dementia.
* Monitor and evaluate patient response to treatment, with autonomy to recommend and implement changes as required.
* Work as a supplementary/independent prescriber according to Trust policies.
* Accept/re-refer/reject referrals to the service as appropriate.
* Refer to other professionals/agencies as required.
* Visit clients and carers in their own home environment to complete initial assessments, reviews, and treatment around care plans.
The role will involve, but not be limited to, performing the above.
What you'll need:
* A minimum of 6 months paid experience in a Community Mental Health hospital environment within the last 3 years.
* Compliance with NMC Professional Registration and keeping up to date with current nursing practices.
*Depending on the Trust, you may require variable additional certification and skills.
