We have a wonderful opportunity for a clinical assessor to join our urgent community response team in Bassetlaw (Retford base). The Community Health Services (CHS) Division of NottsHC is at the forefront of this exciting journey, working with our system partners to create collaborative ways to improve physical health care services in local communities.
Main duties of the job
Urgent Community Response in Bassetlaw is designed to help patients stay out of hospital and support patients in crisis (physical health). We holistically assess patients to enable them to stay at home, aiming to provide the most efficient and effective health care possible via a 2-hour response to patients experiencing a sudden deterioration in their health and wellbeing.
The team enables individuals to increase independence and improve health outcomes by designing programs for recovery and self-care management through timely assessments and interventions. We work in collaboration with patients, their carers, and other statutory and voluntary organisations/agencies, ensuring high-quality care is provided based on the needs of our patients.
The service operates 7 days a week from 08:00 - 22:00 hrs.

These posts are ideal for nurses, physiotherapists, occupational therapists, and other AHPs to join a new and innovative service that focuses on development, innovation, and compassionate care.
You will work as an accountable and autonomous practitioner alongside integrated care teams and acute Trusts to support discharge and prevent hospital admission. You will provide clinical triage and navigation, urgent response assessments, and facilitate discharge from the acute provider.
As a Clinical Assessor, you will:
* Analyse and develop treatment plans within a multidisciplinary framework, providing compassionate support to a diverse patient group.
* Undertake holistic assessments of health and social care needs, incorporating physical health checks.
* Manage a caseload of patients with moderate to complex needs, supporting their rehabilitation and reablement.
* Demonstrate excellent communication and interpersonal skills, establishing effective working relationships with patients and staff.
You must be a Registered Nurse (RGN or RN (Adult)) or Allied Health Professional, with relevant clinical experience and evidence of continuing professional development. A full UK driving licence and vehicle for business use is required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
