This is a new full-time fixed term 12 month post in Telford CRHT in Urgent Care, Shropshire, Telford and Wrekin.
The emphasis is to provide support to the Urgent Care Psychiatrists and the wider multi-disciplinary teams based in Telford to ensure they provide timely and comprehensive assessment, interventions and treatment to people 18+ and their families/carers, who are typically presenting with severe and enduring mental illness, and in a crisis situation, require home treatment or potential hospital admissions.
You would form part of the multi-disciplinary team. The team base is in Telford. The inpatient facilities are based in Shrewsbury at The Redwoods Centre and the Trust also has inpatient facilities in Stafford and Tamworth.
You will participate in Clinical Governance - this includes dealing with complaints, identifying and reporting serious untoward incidents, clinical audit and fostering effective clinical practice.
You will support and provide training/supervision for non-medical staff and students.
You will undertake the medical lead within Clinical Case Workers, providing medical input towards informing and supporting their practice and the delivery of effective care.
Whilst working closely with primary care, enabling qualitative joint working and a smooth transition of care. Where relevant this will include the preparation and writing of summaries and letters regarding care/treatment progress.
The post-holder will offer clinical services to the community working within both local and national guidelines. The postholder will:
1. Need to be familiar with the provisions of the Mental Health Act 1983 and amendments (2007).
2. Clerk new admissions and complete a thorough history and physical examination as well as conduct risk assessments.
3. Under supervision, be responsible for the assessment, management and treatment of patients, including drawing up care plans and providing regular reviews in the community.
4. Under supervision, be responsible for undertaking risk assessments alongside the patient and the wider multi-disciplinary team, and determining and implementing risk management plans.
5. Participate in multi-disciplinary clinical meetings on a weekly basis working closely with the wider multi-disciplinary team and partner agencies as needed such as the local authority.
6. To make regular and accurate notes in patients records as well as update care plans.
7. Participate in the (1:14 approx) first on call rota, for which a 50 % banding supplement is currently paid.
8. As required, cover colleagues who may be on leave or sick.
9. Be required to perform duties in occasional emergencies and unforeseen circumstances without additional remuneration. Commitments arising in such circumstances would be exceptional and you would not be required to undertake work of this kind for prolonged periods or on a regular basis.
