Full job description
The core responsibilities of the Group Practice Nurse are as follows:
1. Develop, implement and embed an effective care management programme for the frail and housebound patients registered within the practices.
2. Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
3. Develop, implement and embed health promotion and wellbeing programmes.
4. Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
5. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
6. Provide routine nursing care to patients in accordance with clinical based evidence, NICE and NSF.
7. Diagnose and manage acute and chronic conditions as required.
8. Prescribe medication for patients in accordance with PGDs and own scope of practice.
9. Review patient medications to enhance compliance.
10. Request pathology services and process pathology results as required.
11. Oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
12. Maintain accurate clinical records in conjunction with existing legislation.
13. Ensure read codes are used effectively by all members of the nursing team.
14. Ensure appropriate use of the chronic disease registers.
15. Prioritise health issues and intervene appropriately.
16. Recognise, assess and refer patients presenting with mental health needs.
17. Support patients in the use of their prescribed medicines or over-the-counter medicines (within the scope of their own practice).
18. Liaise with external services/agencies to ensure the patient is supported appropriately.
19. Delegate clinical responsibilities appropriately, ensuring safe practice and that the task is within the scope of practice of the individual.
20. Support the Data Quality Manager with hitting QOF targets within all practices.
21. Support with the development, implementation and review of all nursing policies and procedures.
