Job Overview The learning disabilities access team manages all referrals to the community learning disability services. Ensuring they are effectively and efficiently screened, prioritised and processed. Access clinicians will ensure that an holistic core assessment will be completed for all referrals accepted to the service by them in keeping with the standards set out within the access pathway and referral exclusion criteria. This will include identification of clinical risk, a full case history relevant to the reason for referral and physical health baseline checks. Clinicians in the access team with review patients routinely who are waiting to be allocated for treatment on a clinical pathway and reprioritise them in response to an updated risk assessment; avoiding harm whilst waiting for treatment. If necessary the Access Team clinicians will escalate concerns and arrange a multi-agency meeting (MAM) to mitigate risks and in turn avoid deterioration of mental and/or physical health; when necessary taking urgent and assertive action to avoid admission. We are a busy dynamic team who are the first point of contact for patients, families, carers and referrers of those accessing our multidisciplinary community team. We have an exciting opportunity to recruit two enthusiastic LD Access Practitioners into our Community Access Team. These positions are open to qualified Occupational Therapists, Physiotherapists, Speech and Language therapists and Nurses. The role provides the opportunities to develop skills and knowledge in Specialist Learning Disability, mentoring students, clinical pathway, and multi-disciplinary work whilst being part of a supportive team. We will provide regular clinical and peer supervision and you will be supported to continue your professional development. The Access Team manage all referrals to the community adult learning disability service. We ensure that referrals are effectively and efficiently screened, prioritised, and processed. Following this, in keeping with the standards set out in the access pathway the Access Clinicians will carry out a thorough holistic assessment of the patient. This will include identification of clinical risk, a full case history relevant to the reason for referral and physical health baseline checks. The Community Learning Disability Team is a Multi-Disciplinary Team, working alongside OT s, Physiotherapists, Speech and Language Therapists, Psychologists and Psychiatrists. We have a Forensic Team and Crisis team who we also work closely with. Additionally, we work closely with our system wide partners in primary care, Adult social care and private and voluntary care providers to identify holistic needs, reduce health inequalities, manage and mitigate identified risks and enable our patients to live their lives as independently as possible in the community with the right support. The post holder will be responsible for: Screening referrals to the learning disability community team to support timely clinical decision making ensuring the service referral criteria are met. Prioritise referrals using the threshold assessment grid, (TAG), including all required risk assessment tools. Undertake core assessments in person, by telephone or virtually to identify individuals health needs, agree an initial intervention plan with patient and/or carers and allocate to the appropriate clinical pathway; formalising the plan with the multidisciplinary team where necessary to secure highly specialised advice. Support the referral of individuals to the Risk of Admission Register (ROAR) and Dynamic Support pathway, (DSP) when required. Work collaboratively with colleagues in the access team to ensure stages in the access pathway are appropriately resourced and supported. Provide information, support and guidance, and be a point of contact for service users, their carers and the referrer to ensure timely response in changes to clinical presentation and to support health and wellbeing. Undertake regular waiting list reviews, (virtually or by telephone) in line with the Access pathway, for all patients waiting for treatment identifying, escalating and mitigating emerging risks as required in order to avoid harm to patients whilst waiting for treatment. All posts are full time and the usual working pattern is Mon-Fri 8.30-4.30 We encourage part time working minimum 0.6 WTE. Flexible working arrangements will be considered in line with LPT policy.
