The post holder will have continuing responsibility for the management of a defined caseload. The post holder will provide a high quality community based general health nursing service for Children in Care & vulnerable children including those working with Sefton Youth Justice Service. This will provide opportunity for integrated and co-ordinated approaches to the needs of the young people. It is envisaged that the main focus of the work will be in the area of health promotion, health screening and clinical intervention.
Candidates must have a minimum of 2 years working with Children in Care.
Main duties of the job
The post holder will support the Lead Nurse to ensure that the organisation meets its responsibilities to Children in Care and will work as a member of the organisation's Children in Care health team at all times and in relation to the roles and responsibilities listed. The post holder will provide comprehensive health screening and assessment for Children in Care on their caseload, and will provide health promotion (family planning, teenage pregnancy, substance misuse guidance) to the target group, as well as provide the target group with access to inclusive primary health care services via dedicated community nursing support (for example, immunisation, fast-tracking access to doctors, dentists, opticians and parenting support). The post holder will practice autonomously and demonstrate evidence based clinical decision making, as well as work within guidelines to identify and manage risk, reporting any identified risks to the Lead Nurse. The post holder will ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

Clinical Governance/Research and Audit

4. Undertake regular caseload supervision with the appropriate specialist nurse & regular clinical supervision.

1. Promote effective communication between the child, carers, family, primary care and other agencies to ensure a cohesive approach to care.

Qualifications
* Be registered on Part 1 of the Nursing and Midwifery Council (NMC) register as a children's nurse or mental health nurse/adult nurse with significant experience working with vulnerable children and young people or Part 3 as a specialist community health nurse.
* Evidence of continual professional development.
