Job summary As a social prescriber you will connect with people from a wide range of backgrounds, taking time to listen and focus on what matters to the patient and take an holistic approach to their needs. You will connect with patients, their carers, community groups and the practice clinicians to provide practical and emotional support to patients with a variety of communication routes including in person, telephone and other multi media methods. This role is initially for 8 hours per week. Main duties of the job To work alongside, clinical and non-clinical colleagues in order to provide non-clinical holistic support to patients. This role will involve a high level of communication skills with multi-disciplinary teams within a primary care GP Practice and across the community. The right candidate will have good listening and verbal skills, who takes a team player approach, has a flexible attitude and always puts the patient at the heart of the service.
