Job summary
Thepost holder will autonomously undertake assessment and treatment of same dayurgent care patients who may present with complex and/or chronic illnessesand/or injury. This will be a portfolio role working across various services,including but not limited to ABC Remote Services (e-Hub and Enhanced Access),and Acute Primary Care Hubs.
Base: Remoteworking within East Surrey, Mid Sussex, Crawley & Horsham. Possibility toattend some face to face clinics within these areas
Main duties of the job
The post holder will determine a clinical diagnosis, treatment and appropriate discharge as indicated as well as maintaining patient records, as well as be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership within a multi-disciplinary team.
The post holder will ensure a high-quality patient centred service is delivered in both face-to-face and remote settings across all clinics.
The post holder should be a Registered first level nurse MSc or equivalent.

Primary duties and areasof responsibility
Clinical responsibilities
1.To independently undertake highly specialisedclinical assessments, diagnose, treat, and discharge patients with a wide rangeof minor illnesses and injuries, including management of complex cases.
2.To ensure patients are fully involved in thedesign and implementation of their treatment plans.
3.To ensure effective coordination and liaisonwith multidisciplinary teams across Primary and Secondary Care Services,statutory and voluntary disciplines/groups to provide a seamless service to thepatients and their carers and prevent hospital admission where urgent care isrequired.
4.To refer appropriately to clinical leads where asecond opinion is required.
5.To adapt styles of communication appropriatelyto promote empowerment, self-management, and health education for patients.
6.To attend clinical case conferences, review, andclinical meetings as required.
7.To be a source of clinical expertise to lessexperienced staff within the team.
8.To delegate duties/tasks to other members ofstaff where appropriate.
9.Provide specialist advice relating to minorillness and/or injury to local healthcare teams, patients, relatives, andcarers.
10. Toidentify actual or potential clinical risks and take appropriate action using,when required, the appropriate reporting structure.
Service Delivery Responsibilities
1.To ensure that patient records and documentationare maintained in line with regulatory standards, ABC policies and use of theelectronic patient records system. This includes:
Documenting full and accurate records ofassessments, investigations, procedures, treatments, and aftercare.
Encouraging colleagues to maintain good andcomplete records.
Working closely with the reception team and theadministrator, ensuring all records pertinent to patients are scanned and filedas appropriate.
Contributing to the development ofcomputer-based patient records
Contributing to the read-coding of patient data.
2.Work within local protocols, guidelines and PGDs(Patient Group Directives).
3.To participate in audits and use the results,current research, and government guidelines to make recommendations for changesto service delivery and clinical practise, and to share knowledge with otherhealthcare professionals.
4.To contribute to the maintenance and improvementof communication networks with all disciplines including community healthcareteams, patients, acute hospitals, and relatives/carers to ensure continuity ofcare.
5.To collect data and provide service informationto the Service Manager as required.
6.To contribute to the development andestablishment of systems to gather feedback from patients.
7.To ensure that service specific Managers andClinical Leads are kept informed on service issues and potential risks.
8.To work with the team in delivering andpromoting the needs of the service.
9.To work with the Service Managers and ClinicalLeads to meet the Clinical Governance needs of the service; includinginvestigation of incidents (including serious incidents) and complaints, whererequired, sharing learning that arises from investigations.
10. Toprovide clinical supervision / mentorship (where appropriately trained) tojunior practitioners.
11. Toremain updated in the use all equipment used in all services and to report anytechnical problems or issues with that equipment.
12. Toparticipate in clinical team meetings to discuss day-to-day operational issues.
