We are looking for a skilled and compassionate Health and Social Care Coordinator to join our team based in Maidstone.
Working as part of our Adult Services team our Health and Social Care Coordinators support patients within each defined primary care network population with social prescribing. The patients will routinely be the top 3% Long term Condition patients with most complex needs.
We take referrals from GP’s, Complex Care Nurses, Community Nursing, Social services, Dementia nurses, Mental health nurses, Specialist nurses/physios, Therapy Leads, Acute Trust and Local Ambulance providers
As a Band 4 Health and Social Care Coordinator you will:
• Support housebound patients living with a Long Term Condition, to effectively manage and improve their well-being.
• Support a reduction in unnecessary hospital attendances, and subsequent admissions.
• Support hospital discharges from Acute and specifically Frailty Units.
• Respond to referrals within an appropriate time frame, and to refer to other services as appropriate. (pathway doc)
• Support inter service referrals.
• Provide advice and support to patients, carers and staff.
• Provide support to patients being discharged from clinical staff, for step down where further support recognised, despite health needs met.
• Manage a caseload of patients, plan expectations, and recognise outcomes.
• Create Personalised Plan for each patient, and support MDT with advocacy role.
• Become the responsible person for MDT journey, including outcome & actions review as per pathway.
• Ensure good communication to lead.
