They will have experience of working with individuals with severe and enduring mental illness, including having managed their own caseload. The post holder will be able to build relationships and work closely with local community Pharmacists and General Practitioners within the Primary Care Network. This is a specific yet rewarding role and will enable the successful applicant to work within a multi-disciplinary community team and have direct patient contact. The ideal candidate will be within a multidisciplinary community mental health team (CMHT), working closely with specialist clinicians to optimise medication and improve treatment outcomes for people with mental health conditions. You will receive professional support via a well-structured system of supervision and regular educational and CPD sessions. The successful applicant should be competent in auditing, undertaking, action planning and presenting projects in line with targeted quality indicators. Working Monday to Friday 9am 5pm and requires travel across a large geographical area, access to suitable transport is required.
