Job summary An exciting opportunity has arisen within this Specialist Continence & Urology Paediatric Service. You will join an establish team of Nurses, Nurse Associates and Healthcare Workers, delivering care to Children and Young People across West Cheshire. Care will be provided in a multi-disciplinary setting; Clinics, Schools, Social Care setting across West Cheshire Footprint Provide Toileting Assessments, advice and support that enables children, their families and carers to maximise their health, well being and potential. Provide reassessment of care plans and advice provided to children, their families and carers and ensure Care Plans meet their needs. This vacancy is part time 22.5 hours per week. Main duties of the job 1. Provide help and support to parents to enable them to make informed choices and support Toilet Training and other Continence issues 2. Liaise with multi-agency professionals as appropriate 3. Understand how to use and maintain equipment safely and competently. 4. Maintain accurate records and store in line with Trust Policy 6. Participate in Meetings as required, ie Safeguarding 1. Provide telephone support as required. 3. Store and maintain clinical records in accordance with Trust Records Policy. 4. Report sickness and absences to Clinical/Service Lead 6.Provide accurate and timely information to relevant staff regarding annual leave, expenses and study leave. 7. Report to facilitator Service Lead or Clinical Lead, all accidents and incidents and complete necessary documentation. Communication 1. Communicate with relevant team members regarding client progress and any specific or urgent concerns. 2. Communicate and collaborate with other professionals within CWP West and other voluntary or statutory organisations involved in providing support for individual clients and/or families where appropriate. 3. Support the development of integrated working within CWP West Starting Well Team. Support the development of student nurses as appropriate. Contribute to public health agenda. Training and Education.
