Specialist Parkinson's Practitioner - Nurse or AHP
To assess, plan, implement and monitor care, facilitating best practice and evidence-based, person-centred care within Parkinson's. To facilitate patient empowerment by enabling people with Parkinson's to manage their condition effectively and independently, preventing complications and improving quality of life. To be a resource and provide specialist advice, education and support to other health care personnel and people with Parkinson's, and contribute to planning, delivering and evaluating programmes of education and development for Parkinson's care. To be a lead in monitoring and developing the Parkinson's service, to ensure patients receive sustainable high-quality care that is effective and safe. Ensure a seamless service for the person with Parkinson's by developing communication links and partnership working between acute, community, primary care and voluntary services. Provide advice, training and education to others within Sirona and in the wider health and social care system.
The current team vacancy requires flexible cover, to include all of the BNSSG footprint.
Main duties of the job
1. Provide high quality, comprehensive and timely assessments for people with Parkinson's in order to support joint goal setting at all stages of Parkinson's including end of life care.
2. Offer specialist advice, training, and education to GPs, community teams, specialist services, and wider health and social care about Parkinson's.
3. Use specialist knowledge to refer to other professionals and for diagnostic tests.
4. Provide psychological and emotional support to service users, families, and carers in high-stress situations and use counselling skills.
5. Take responsibility as an independent non-medical prescriber to prescribe medicines from the relevant national formulary for individual patients receiving care for a condition for which the HCP has clinical responsibility.
6. Reduce the need for acute care and emergency admissions and facilitate early hospital discharge by liaising with hospital teams and ensuring appropriate support and monitoring of people with Parkinson's in the community.
7. Participate in shaping, developing, reviewing, and improving the Parkinson's service in Sirona.
8. Represent Sirona care & health on external committees and relevant groups.
9. Lead on the development and review of local policies and procedures relevant to the role.

Person Specification
Qualifications
* Registered Nurse or health care professional
* Bachelor's degree in nursing or health-related subjects or equivalent experience
* Non-medical Prescribing course or willingness to complete.
* Evidence of continuing professional development
* Teaching and mentoring qualification or equivalent experience.
* Specialist courses regarding the management of Parkinson's disease patients or willingness to complete.
Experience
* Excellent verbal and written communication skills able to identify and overcome barriers to understanding, with empathy and reassurance.
* Ability to develop and manage own case/workload
* Evidence of professional development and regular study.
* Ability to critically analyse research.
* Evidence of experience in managing Parkinson's disease.
* Experience of team and partnership working
* Ability to interact with a wide range of stakeholders including third sector and senior management
* Experience of community working
* Experience of teaching/education/leadership
