Read coding the information into the practice clinical system and monitoring progress of notes summarising against practice targets.
3. Long Term Condition Invitation: Coordinate the practice system for inviting patients for appointments for their annual health reviews and medication monitoring. Send invites and reminders ensuring most vulnerable patient groups are prioritised. Work with other teams to ensure their caseloads are known and being worked through.
