Job summary
The Essex Family Wellbeing Service is seeking a dedicated and detail-oriented Supporting Families Practitioner to review patient health records and ensure their alignment with the National Supporting Families Outcome Framework.
The role involves validating health data against the framework's criteria to support comprehensive family assessments and ensure accuracy in data reporting and service provision.
This role is Fixed Term until 30th June 2025.
The role is available across Essex. You will be based in one of our Family Hubs that will be local to you.
Main duties of the job
The post holder will support the delivery of an effective service through the provision of support in the areas described-
1. Conduct systematic reviews of patient health records to identify relevant data that aligns with the National Supporting Families Outcome Framework.
2. Ensure the accuracy and completeness of health information in relation to the framework's outcomes and indicators.
3. Conduct systematic reviews of patient health records to identify relevant data that aligns with the National Supporting Families Outcome Framework.
4. Complete reports summarising the alignment of health data with the framework's criteria.
5. Work collaboratively with system partners, to integrate relevant data into Patient Health records to accurately support National Supporting Families Outcome Framework.
6. Adhere to the service referral protocols. Following processes regarding escalation of cases where the level of need becomes beyond scope.
7. To identify support needs, and provide signposting for patients and carers families
8. To participate in networking with all other relevant areas to enable a seamless pathway of care - Stay informed about updates to the Supporting Families Outcome Framework and best practices in health data management.
9. Engage in ongoing training and professional development opportunities related to family support and health record analysis.

The Ideal Candidate
15. GCSE English and Maths, Grade C and above, or equivalent qualifications.
16. Experience within an administrative role or experience of working in a customer service environment.
17. Excellent communication and interpersonal skills, both verbally and written, to all levels both internally and externally.
18. Effectively prioritises and coordinates own work alongside work of others
19. Standard keyboard skills
20. Ability to use Microsoft Office packages
