Job summary We are looking for a Band 4 Older Persons Practitioner who may be an Assistant Practitioner, Nurse Associate or someone willing to work towards this who has significant experience in this sector. You will be working as part of the wider health and social care team and have direct contact with patients, service users or clients in providing high quality and compassionate care. Our Band 4 Practitioners work at a level above that of healthcare support workers and have a more in-depth understanding about factors that influence health and ill-health. Our Band 4 Practitioners work in a variety of our services such as: The Older Persons Community Mental health team completing physical health checks and reviews of our patients on antipsychotic medication as well as supporting keyworkers with developing treatment and management plans for our patients. Dementia Assessment Service by completing physical health checks and reviews for our patients on memory medications as well as completing post diagnostic visits. Assisting in delivering therapeutic groups, such as living well with dementia and cognitive stimulation therapy, in our community venues. This role will include completing post-diagnostic dementia visits and reviews for our patients on memory medications but will be utilised within the other areas in the older persons team as required. Main duties of the job Experience of working in a care setting is essential but all necessary skills training, professional updates and mandatory training as appropriate to the post will be provided.
