About The Role
Are you an experienced GP, ANP or ACP looking to be part of a successful and developing in and out of hours service?
Here at Practice Plus Group, we are looking for experienced Primary Care GP’s, ANPs, ACPs to work within our expanding Devon Care Coordination Service on both bank agreements or a contracted basis.
What is Devon Care Coordination?
The Care Coordination Hub currently works alongside the South Western Ambulance Service and community services to provide non-ED pathways and reduce avoidable ambulance attendances and transport through access to community-based services and provision of clinical advice.
The service operates from 8am to 8pm, 7 days a week and is based on-site within PPG’s Devon HUB at Stratus House Exeter. All shifts are completed by telephone from the Care Coordination Hub.
What you'll be doing
What you will need:
As an ANP or ACP:
* Recent experience in telephone triage as an autonomous advanced practitioner in a primary/urgent care environment, with a proven ability to see, treat and discharge patients of all ages with unscheduled presentations.
* A master's level qualification including patient assessment and independent prescribing and professional registration with either the NMC or HCPC.
* Excellent listening and communication skills, with good IT and keyboard skills.
* Experience of working with other agencies and an understanding of the health infrastructure within Devon would be advantageous.
As a GP:
* GMC registration certificate and inclusion on the GP Performers List with a valid CCT.
* Experience to confidently manage acute and chronic presentations for both adult and paediatric patients, with a proven ability to maintain and monitor high standards of care.
* Excellent listening and communication skills, with experience of urgent care assessment and good IT/keyboard skills.
