Job summary We are looking for talented and dynamic Therapy Assistant with relevant and transferable experience in a health or social care role. The service works with people living with Dementia in their own homes. The aim of the service is to enable people to live will with Dementia whilst prevent crisis by identifying potential risks and working with the person and their support network to put pro-active management plans in place. You will be working as part of a multi-disciplinary community team including Nurses, Occupational Therapists, Healthcare Assistants, Dementia Coordinators and Administrative staff. You will also work closely with other community health and social care teams, voluntary services and acute frailty teams to formulate care plans with people living with Dementia. Main duties of the job The service is delivered within the Hounslow community with visits being provided within patients own homes. The post holder will work flexibly and be able to travel across the Hounslow Borough and to health or social care sites as required. The service operates 7 days a week and all staff are expected to work across 7 days on a rota basis. As a Therapy Assistant you will work with the multidisciplinary team and specifically alongside Occupational Therapy to deliver the following plus additional duties as you develop within the role: o Provide autonomous assessment of functional components such as mobility, transfers, daily activities or a person's environment following a full initial assessment by the teamo Deliver therapy or rehabilitation plans with patients including their care and support network where appropriateo Demonstrate and apply knowledge of community risks in the context of Dementia, communicating relevant observations back to the teamo Complete relevant training and service competencies in order to deliver core functions including vital signs, NEWS2 scoring, skin checks, hydration and nutritional status checks About us West London NHS Trust is one of the most diverse healthcare providers in the UK, delivering a range of mental health and physical healthcare and community services.
