Are you an experienced social worker looking to take the next step in your career within an integrated team? We have an exciting opportunity for a Senior Practitioner to join our Integrated Intermediate Tier Services at Warrington General Hospital. In this role, you will be part of the Transfer of Care Hub and the Intermediate Care Bed-Based Team, where you will have supervisory responsibilities and work within a truly integrated system. The position offers 37 hours per week, and unfortunately, part-time hours cannot be considered.
Recovery in a community or home setting is often the best way for individuals to continue their healing process after a hospital stay, reducing unnecessary hospital time for those who are more suited to other care environments. While many people can return home without additional support, others may require further assistance, from short-term rehabilitation to long-term care. Support may include:
1. Rehabilitation and recuperation at home or in a bed-based setting, with assistance ranging from days to months until full recovery or maximum potential is achieved.
2. Returning home with a care package, including services, aids, or necessary home adaptations.
3. Long-term support in a care home, often for the rest of their lives.
A safe and effective hospital discharge process is crucial for enabling individuals to regain their independence at the appropriate time. At Warrington and Halton Teaching Hospitals NHS Foundation Trust (WHH) and Warrington Borough Council (WBC), our Transfer of Care Hub has undergone a transformation, moving beyond co-location to foster integration across our services. Together, we are developing a comprehensive system to deliver the health and social care services of tomorrow, today.
The role
As a Senior Practitioner, you will play a critical role in the Transfer of Care Hub, providing supervision and guidance to your colleagues while working collaboratively with health and social care professionals, carers, families, and other statutory and voluntary organisations to oversee and plan the support individuals need to return to the community. Your leadership will be vital in ensuring that the team adopts a solution-focused approach, placing the person’s needs at the heart of every decision to help them not only return to the community but also live well and independently.

What's needed?
Your role will require strong collaborative working and the ability to maintain effective relationships with a wide range of health and social care colleagues, both in the hospital and the community. You will operate within the Care Act framework, ensuring the individual’s needs are at the centre of all assessments and decision-making, while evaluating and managing risks in partnership with professional colleagues. As a Senior Practitioner, you will also support the development of team members, providing regular supervision and mentoring to enhance their skills and practice.
