Salisbury Medical Practice is looking to recruit a committed, caring and highly motivated Pharmacist to join our well-established Medical Practice.
Main duties of the job
As a Pharmacist, you will act autonomously within your professional boundaries to provide support to the GPs and the repeat prescribing team in all medicines management matters and in implementing effective medicine management within the Practice and identifying areas for improvement. The Pharmacist will work as part of a multi-disciplinary team along with fellow Pharmacists and Pharmacy Technicians and will be in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
We are looking for someone who is a qualified and competent Pharmacist and desirably has previous experience working within a General Practice environment. Applicants must have excellent organisational and communication skills as well as a high level of competency using computers. The successful candidate will have the ability to liaise with staff at all levels and work effectively as part of a team. A strong commitment to personal and professional development and teamwork are essential for this role. The applicant must be a qualified, registered independent non-medical prescriber (or working towards/intent of gaining independent prescribing qualification).

This is a permanent position and we are open to full-time or part-time (minimum of 18.75hrs per week) applicants; desired hours can be discussed at interview. Job responsibilities
* Act as the first point of contact to provide specialist expert knowledge in the use of medicines and for all prescription queries and medication requests from staff and patients.
* Undertake clinical medication reviews with patients where medicine optimisation is required to reduce waste and promote self-care.
* Reconcile discharge summaries and medications for patients being discharged from hospital and/or moving into a care setting.
* Be responsible for the care management of patients with chronic diseases and undertake medication reviews to proactively manage patients with complex poly-pharmacy (including but not limited to the elderly, care home residents, co-morbidity patients, etc.).
* Implement changes to medicines that result from MHRA alerts, product withdrawal, and other local and national guidance.
* If an independent prescriber, to prescribe, monitor, and review medication that is appropriate to patient needs and in accordance with the latest evidence-based practice, national and practice protocols, and within the scope of qualified practice.
* Undertake CPPE training as identified.
* Ensure patient confidentiality, respect, and dignity are adhered to at all times.
* Produce recommendations for the GPs and other healthcare providers within the Practice on prescribing and monitoring.
* Help produce, review, and implement a Practice repeat prescribing policies and procedures.
* Identify cohorts of patients at high risk of harm from medicines and put in place changes to reduce the prescribing of medicines to these high-risk patient groups.
* Undertake clinical audits of prescribing and feedback the results to implement changes.
* Assist Partners with the monitoring and management of their prescribing budgets.
* Audit Practice compliance against NICE technology assessment guidance.
* Work with the immediate and wider team to ensure that the Practice is compliant with CQC standards where medicines are involved.
* Provide lifestyle advice to patients making any necessary referrals within the Practice.
* Act as a chaperone at the patient or GP request and annotate the records thereafter.
* Use Ardens templates to ensure accurate and legible notes of all consultations and treatments are recorded in the patients' records.
* Contribute to local and national practice targets e.g. QOF, national benchmarking, etc.
* Ensure clinical practice is safe and effective and remains within boundaries of competence, and acknowledge limitations, embarking upon further training if required.
* Deliver high-quality, compassionate care and treatment to patients with a focus on promoting health, self-care, and independence.
* Take responsibility for own development, learning, and performance to ensure own educational commitment is at least sufficient to maintain registration requirements.
* Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.
* Undertake statutory and mandatory training as required.
* Act with professionalism and ensure that ethical conduct is adhered to at all times.
* Assist with training for new staff.
* Assist with the gathering of statistics and information when required.
* Participate and co-operate with any research projects.
* Work across different practices on an ad-hoc/when required basis.
* Attend and contribute to various practice meetings as requested.

Person Specification
Qualifications
* Pharmacy degree (or equivalent), registered with the GPhC, qualified, registered independent non-medical prescriber, or willing to attend training.
Experience
* Has 2 years plus trained Pharmacist experience and also has experience in undertaking medication reviews.
* Has 4 years plus trained Pharmacist experience and also has experience in working in General Practice or primary care.
Knowledge
* Demonstrate sufficient knowledge of the English language to communicate effectively with staff and patients.
* Has knowledge of clinical computer systems- SystmOne (TPP), Ardens templates, knowledge of NICE guidelines, and QOF. Has an understanding of prescribing budgets and financial information. Has worked on Microsoft Windows Applications - Outlook.
Qualities
* Is an independent worker / autonomous and can also work collaboratively in a team with the ability to multitask. Has a flexible approach to demand and is calm with a professional disposition.
Skills
* Completed all aspects of the application form with no spelling mistakes or errors. Competent and confident in using IT & computers. Excellent communication skills both verbal and written and excellent organisational skills with the ability to prioritise workload.
