About The Role
We are seeking a dedicated and knowledgeable Clinical Pharmacist specializing in Home Parenteral Nutrition (HPN) and Small Volume Parenteral Nutrition to join our dynamic team. The successful candidate will play a crucial role in ensuring the safe, effective, and efficient delivery of pharmaceutical services related to parenteral nutrition therapy.
This is a hybrid role based from our site in Harlow, Essex.
Key Responsibilities:
Patient Assessment and Nutritional Needs:
Assess patients' nutritional requirements and develop personalized parenteral nutrition (PN) plans.
Collaborate with healthcare professionals to ensure comprehensive patient care.
PN Formulation and Quality Management:
Design, compound, and dispense PN formulations.
Ensure the quality and stability of PN formulations, adhering to best practices and regulatory standards.
Monitoring and Supervision:
Monitor patients' responses to PN therapy, adjusting formulations as necessary.
Supervise home parenteral nutrition (HPN) programs, ensuring patient safety and adherence to protocols.
Prescription Management:
Accurately screen and clinically check prescriptions before dispensing.
Manage prescriptions to ensure safe and efficient service delivery, including checking for potential drug interactions and ensuring correct dosages.
Professional Standards and Ethics:
Maintain professional standards and adhere to the code of ethics.
Engage in ongoing professional development to stay current with new medications, treatment guidelines, and regulatory changes.
Collaboration and Communication:
Work collaboratively with internal and external stakeholders to maximize productivity and effectiveness.
Maintain open and transparent communication with the oncology coordinators, nursing team, and other healthcare professionals.
Key Measures of Success:
Competence in all therapies and medications dispensed.
Accurate screening and clinical checking of prescriptions.
Effective management of prescriptions to ensure safe and efficient service delivery.
Positive feedback from management and stakeholders.
Achievement of department targets and KPIs.

About You
* Registered and qualified Pharmacist recognized by the General Pharmaceutical Council (GPhC).
* Knowledge of specialist therapy areas, particularly in parenteral nutrition.
* Experience in homecare pharmacy, hospital pharmacy, or a relevant environment.
* Thorough understanding of the legal and regulatory framework governing pharmacy practice in the UK.
* Excellent verbal, written, and presentation skills.
* Ability to build professional relationships with pharmacy technicians, healthcare professionals, and other colleagues.
* Meticulous attention to detail and adherence to safety protocols.
* Commitment to ongoing professional development and staying current with the latest developments in pharmacy practice.
