About the Role This unique position combines the responsibilities of a Discharge to Access (D2A) Care Assistant with a broader care role, offering you the chance to assist customers in their rehabilitation and daily living needs in the comfort of their homes. You'll work closely with healthcare professionals, including occupational therapists, to design and implement person-centred care and rehabilitation plans. You will also be involved with daily customers who need support in their own home and who need our services on a daily basis. Key Responsibilities Provide safe, reliable, and compassionate support tailored to individual rehabilitation and care needs. Collaborate with the care team, hospital discharge staff, and occupational therapists to ensure smooth transitions and recovery. Assist customers with personal care, medication management, meal preparation, and light housework. Engage customers in social, physical, and mental activities to promote well-being and independence. Safely use aids and equipment while respecting customers' dignity. Monitor and document customer progress towards rehabilitation and care goals, reporting any changes or concerns promptly. What You’ll Bring A compassionate nature and excellent communication skills. The ability to work independently while maintaining strong collaboration with healthcare professionals. A full UK driver’s license and access to reliable transportation. A clear understanding of safeguarding, health and safety, and when to seek help to ensure customer well-being. Previous care experience is a plus, but comprehensive training is provided for the right candidate. What We Offer Competitive Pay: £13.45 per hour, plus £11.44 per hour travel time and 35p per mile.
