Some of your duties will include: Working within the primary care team, you will work with Nurses and Nursing Associates across the service Assist with clinical observations, vaccination clinics etc Provide outstanding quality based care to all patients Deliver personalised, non judgemental care, based on patient feedback Contribute to, and implement care packages, assist with health promotion, wellbeing and clinics Support the nursing and pharmacy teams onsite Support the mental health and substance misuse teams as necessary Assist with release and discharge when needed If you are interested in career development, we have a wide range of opportunities for you to develop your skills and experience, which include both internal and external options for development and learning.
